BRIGHT SMILES FOR EDUCATION

This is a new experience in the life of the Foundation!  Dr. John Phillips of 29th Street Dental Care : ) has graciously donated his time and staff to accommodate a fundraiser for CPSF for the betterment of the students of Chickasha.  For your donation of $125.00 to the Foundation, donors will receive a $325 retail valued Customized Teeth Whitening.  Dr. Phillips professional team will work with each donor to provide the best teeth whitening solution.  The teeth whitening includes a specialized tray impression of your mouth.  The date of the event is Friday, August 27th, 2010 from 8:30 to noon to have your trays formed.  Dr. Phillips office will work with the donor to facilitate their return for the customized teeth whitening trays.  Supplies are limited and reservations must be made no later than Wednesday, August 25th, 2010.  To make reservations for your teeth whitening please call Dr. Phillips office at 29th Street Dental Care at (405) 222-0222.  Dr. Phillips office is located at 2900 W.
